1. Fill out the form below with your flight information and details to ensure that you are compliant with US Airways™ flight requirements before your travel day.
2. Print your Guaranteed on Board™ Certificate and bring with you to the airport and present to the boarding agent.
3. If you are denied boarding due to the Sherpa Pet Group carrier that you are using to transport your pet, the Sherpa Pet Group will reimburse you for the cost of your flight and your pet fee, subject to the rules and conditions listed below.

Carry-on pets are not allowed on transatlantic flights or flights to/from Brazil, Tel Aviv, Barbados and Hawaii.
Only dogs and cats are allowed to/from Mexico and Costa Rica.

The number of pets that may be carried in each cabin is limited.
Pets in cabin count toward the carry-on baggage allowance.

Carrier Information
Pets must remain inside their approved container under the seat directly in front of you.
Carriers must be leak-proof and escape-proof.
Carriers must be ventilated on at least 2 sides but not allow any part of the animal to protrude outside of the container.
Carriers must provide room for the animal to stand up and turn around.

If you and your pet are denied boarding the aircraft due to any issues other than those related to the carrier, this offer is void
If your pet is viewed as a threat to the safety of you, itself or others, this offer is null and void
If the information that was filled out and submitted is not accurate, this offer is void
If you fail to arrive at the gate on time and are denied boarding, this offer is void
If the carrier that you are using is damaged, in poor repair or condition, this offer is void
If the safety of your pet is in question due to non related carrier issues, this offer is void
